We make every effort to keep down the cost of your medical care. Therefore, payment in the form of cash, check or credit card is expected at the time services are provided. This includes any co-pays that may be due as directed by your insurance plan. If you need to make special payment arrangements, our billing department will be happy to help you. Due to the rapid changes in healthcare, we encourage you to call the office for the most current insurance information.
If you have insurance or are enrolled in a managed care insurance program, please bring your insurance card to every appointment. We participate with several managed care programs and since these programs have special provisions regarding where testing and consults are referred, we need to know your plan type.
Failure to identify yourself as a plan participant will result in you being financially responsible for the care that otherwise would have been covered under your program.
Please remember that insurance is considered a way of reimbursing the patient for fees paid to the doctor. It is your responsibility to pay any copays, or deductible amount, or other balance not paid by the insurance company.
