Seung-ha “Grace” Eltringham, FNP-BC, is a board-certified Family Nurse Practitioner providing comprehensive primary care across the lifespan. Her clinical focus includes preventive care, acute care, women’s health, and chronic disease management, with an emphasis on helping patients maintain long-term health and quality of life.
Grace has extensive experience in internal medicine across multiple settings, including hospital-based care, outpatient primary care clinics, and skilled nursing and long-term care settings. She has cared for patients with a wide range of conditions, including hypertension, diabetes, COPD, and acute illnesses. She is dedicated to providing personalized, patient-centered care and takes a proactive approach to prevention, early intervention, and timely treatment.
Originally from South Korea, Grace brings a thoughtful and culturally aware perspective to her practice and values building strong, trusting relationships with patients from diverse backgrounds.
She is board-certified as a Family Nurse Practitioner by the American Nurses Credentialing Center (ANCC) and has also successfully completed national certification through the American Association of Nurse Practitioners (AANP). She is licensed to practice in Pennsylvania, West Virginia, and Ohio.
Grace’s approach to care is rooted in listening first, clear communication, and developing practical treatment plans that align with each patient’s goals and lifestyle.
Outside of her clinical work, Grace enjoys traveling, trying new foods, and experiencing different cultures, as well as spending time with her family and staying active outdoors, including hiking.
